That’s why we offer the KD Membership Plan, a private in-house membership plan designed to help you save on a long list of state-of-the-art services.

This private dental savings plan is NOT an insurance plan and it is only valid at our Scottsdale, AZ dental office.

How Much Does the KD Membership Plan Cost?

  • Prophy Patients – $449
  • Periodontal Patients – $599•
  • Children – $199

Prophy/ Children Plan Benefits:

  • Routine exams (2 per year)
  • Dental cleanings (2 per year)
  • FMX/Panoramic X-rays (1 per 5 years)
  • Bitewing x-rays
  • Fluoride
  • Emergency Visit (1 per year)

Periodontal Plan Benefits:

  • Routine exams (2 per year)
  • Perio Maintenance cleanings (3 per year)
  • FMX/Panoramic X-rays (1 per 5 years)
  • Bitewing x-rays
  • Fluoride
  • Emergency Visit (1 per year)

20% Savings on:

  • ROOT CANALS, FILLINGS, CROWNS, BRIDGES, EXTRACTIONS, SCALING AND ROOT PLANING, VENEERS, ETC… (EXCLUDES INVISALIGN)

10% Savings on:

  • ALL IMPLANT & DENTURE PROCEDURES

  • THE KD MEMBERSHIP PLAN IS NOT INSURANCE.
  • PAYMENT IS ALWAYS DUE AT TIME OF SERVICE.
  • CARECREDIT IS NOT ACCEPTED AS A FORM OF PAYMENT.
  • IT IS THE RESPONSIBILITY OF THE MEMBERSHIP OWNER TO MAXIMIZE THEIR BENEFITS BY ARRANGING ALL APPROPRIATE APPOINTMENTS WITHIN THE 12-MONTH MEMBERSHIP PERIOD.
  • IF BENEFITS ARE NOT USED, THE MEMBER WILL NOT BE ENTITLED TO A REFUND.
  • OUR DENTAL MEMBERSHIP PLAN BENEFITS ARE NOT TRANSFERABLE TO ANY OTHER DENTAL PRACTICE.
